Everyone forgets things from time to time. This short validated questionnaire helps you notice how often those small lapses happen. You can also answer for a loved one, so you can decide whether they are worth a conversation with a doctor.
This is a screen, not an assessment. It does not diagnose dementia, Alzheimer's disease, mild cognitive impairment or any other condition. It is a structured way to reflect on everyday slips, on your own or with help from a family member or carer, so you have something concrete to discuss with a health professional.
